Information
If you are ordering directly from Immuno-Mycologics,
by email, fax or conventional mail, please include
the following to place your order:
1. Purchase order number (VISA and Mastercard
credit cards may also be used for purchase. Include:
card number, name on card, and expiration date)
2. Shipping and billing addresses
3. Also supply, product catalog number,
description, quantity and price of products to
be ordered.
4. Also required: names, phone numbers,
fax numbers, and email addresses of purchasing
agent and end user
5. Quotation number, if applicable
6. Name of Carrier and account number,
if you prefer a Carrier, other than UPS. If desired,
we will email or fax current product pricing and
estimated shipping charges, prior to shipment.
Please indicate on your order, if you desire this
service.
Minimum order $25
Written confirmation of orders is not required
or recommended. If a confirming order must be
sent, please mark clearly on the order: "Confirming
Order Do Not Duplicate." If a duplicate shipment
is received, please contact our Customer Service
department immediately for instructions.
Special
Orders
Immuno-Mycologics is able to provide custom services
and special quotes for bulk products. Prices and
terms are determined on a case by case basis unless
specific arrangements are made for standing orders.
Special orders require a quotation for fulfillment.

Please note that special orders are produced on
a best-efforts basis. Immuno-Mycologics cannot
provide performance guarantees for applications
which we have not verified.
Payment
Terms - (INVOICES FOR PURCHASE ORDERS):
Net 30 with a 2% discount if invoice is paid in
20 days, with payment to be in U.S. currency.
Prices may be subject to change without notice.
Overdue charges, for invoiced amounts over 30
days from invoice date, are billed at 1.5%/month.
Shipping
Orders are shipped on the day received, if before
2:00 p.m. (CST), Monday through Thursday. Shipment
is by United Parcel Service (unless otherwise
specified) for overnight or 2nd day delivery,
with shipping charges (F.O.B. Norman, OK) prepaid
and added to the invoice.
Returned
Goods
Please obtain a Return Merchandise Authorization
(RMA) Number from our Customer Service department
prior to return. Requests for credit should be
made within 14 days of receipt of order. Products
which do not perform according to stated manufacturing
claims may be replaced or returned for credit.
Returns or replacements cannot be accommodated
for expired products. All products sent without
an RMA Number will be returned to sender. Products
returned due to customer error will be charged
a 25% restocking fee.

Immuno-Mycologics, Inc. has a Total Quality
Assurance Policy. Our TQAP guarantee is:
"if a reagent or kit does not perform as stated
in our product literature, we will promptly process
a replacement or refund". All products are manufactured
to strict guidelines and industry requirements;
this high degree of dedication will allow you
to achieve quality, reproducible results for the
applications stated in the product literature.
Immuno-Mycologics' Total Quality Assurance Policy
is a customer's exclusive remedy for any defects
or deficiencies in the products and for Immuno-Mycologics'
performance. THERE ARE NO EXPRESS, IMPLIED OR
STATUTORY WARRANTIES, INCLUDING WITHOUT LIMITATION,
THE IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS
FOR A PARTICULAR PURPOSE AND NONINFRINGEMENT OF
THIRD PARTY RIGHTS. IMMUNO-MYCOLOGICS SHALL NOT
BE LIABLE FOR ANY INCIDENTAL, INDIRECT, SPECIAL
OR CONSEQUENTIAL DAMAGES, EVEN IF IT IS AWARE
OF THE POSSIBILITY OF SUCH DAMAGES. IMMUNO-MYCOLOGICS'
TOTAL LIABILITY FOR ANY ORDER SHALL NOT EXCEED
THE AMOUNT PAID BY CUSTOMER UNDER SUCH ORDER.
The terms and conditions in this document constitute
the entire agreement between the parties with
respect to the products purchased hereunder. Any
additional, different or inconsistent terms and
conditions in a purchase order form or like forms
used by customer to purchase, change, accept or
otherwise process orders are objected to and not
binding on Immuno-Mycologics.

Discounts are available for large quantity, bulk
or standing orders. Please contact the Marketing
Manager for details.
